Graham Healthcare Group is hiring a Palliative Patient Specialist to join our dynamic team! The Palliative Patient Specialist will partner with our field staff and leaders to provide the best support to the patients that we serve.

Compensation: $19.00 – $21.00  The base compensation range for this role is fixed, with a maximum cap of $21.00. We want to be transparent about this as we continue discussions.

Palliative Patient Specialist Job Responsibilities:

  • Develop and nurture referrals while ensuring accurate documentation.
  • Manage various communication channels for the team, including Teams, emails, and Salesforce.
  • Serve as a primary contact, advocate, and source of information for patients and families
  • Maintain an updated list in Salesforce, including non-admitting/discharging charts and patient assignments.
  • Demonstrate excellent customer service, both internally and externally.
  • Understand and effectively communicate HIPAA compliance, corporate compliance, and client confidentiality.
  • Manage a census of patients who are not current with another service line offered by Residential. This may include patients discharged from Home Health, Journey, Palliative, or Hospice; patients whose Hospice start of care was delayed by patient choice; and patients needing an in-person phone consultation regarding levels of care as requested by another service line across the continuum.
  • Perform screenings/needs assessments over the phone to determine whether patients meet the eligibility requirements for levels of care.
  • Perform educational sessions over the phone to maintain relationship with patients and/or MPOA as dictated by patient needs.
  • Communicate and collaborate with Palliative, Home Health, Journey and Hospice teams as needed to expediate access to care and internal team collaboration
  • Serve as a liaison between the company and patients/caregivers
  • Respond to company web-based inquiries for service information as needed
  • Comply with Company’s Core Values and Core Competencies
  • Perform other duties as assigned by Supervisor and/or Director
  • Ensure compliance with local, state, and federal regulations.

Palliative Patient Specialist  Qualifications/Requirements:

  • High School diploma/GED
  • 2 years of Hospice and/or Home Health experience preferred
  • Medical Terminology knowledge preferred
  • Active listening skills
  • Customer service skills
  • Possess patience, dependability, good and sound judgement, and the ability to manage concurrent demands
  • Proficient in Microsoft Office suite including Teams

Palliative Administrative Specialist​ Responsibilities:

  • Coordinate, schedule, and assign palliative patients to providers

  • Conduct follow-up calls to patients and families to collect data about patients/family response to the treatment plan established by the assigned provider

  • Serve as the contact point, advocate, and informational resource for patients, care team, family/caregiver(s), payers, and community resources

  • Manage referrals, when needed, to appropriate agencies required to assist the client in achieving the goals and objectives identified in their Care Plan

  • Assist patients in problem-solving potential issues related to the health care system, as well as financial or social barriers (e.g., request interpreters, transportation services, or prescription assistance as appropriate)

Palliative Administrative Specialist​ Requirements:

  • A minimum of 2 (two) years of experience in Palliative, Hospice, or Home Care is required

  • Bachelor’s degree in a health-related field preferred

  • Medical records experience preferred

  • Experience using an applicant tracking system to both maintain candidate workflows and track key metrics
  • Demonstrated success recruiting in difficult to fill markets
  • Experience operating as a strategic partner to the business and building consultative relationships with key stakeholders
  • Demonstrated ability to yield consistency in quality of hire through effective interviewing practices
  • Proven success meeting/exceeding recruiting metrics
